The concept

Whole-Face Design
as a Way of Thinking

One millimeter changes the impression

Consultations about aesthetic medicine usually begin with a single area. "I want a higher nose." "I want a slimmer contour."

But a face is not a row of independent parts. Each one affects the others, and together they form "one structure" held in delicate balance.

That is why KAI CLINIC TOKYO treats your consultation as the "design of the whole face" and works with you to identify only the procedures that are truly necessary. Here we introduce the approach we value: whole-face design.
01 The starting point

A feeling for the "one millimeter"

What we handle in cosmetic surgery is, in most cases, changes measured in millimeters, sometimes less than one. A difference of a single millimeter changes the impression of a face remarkably, and with it a patient's habits of expression and the atmosphere they give off.

Because such small changes so greatly influence the impression of the whole face, we have specialized in facial surgery and approached it with advanced skill and care.

We do not see a face as a mere "collection of parts." The height of the nose affects how the eyes look, and the contour line takes its impression from its balance with the nose and mouth. When one spot moves by a millimeter, the change always affects the balance with the surrounding features.

An "ideal shape" and an "ideal impression" are not the same.

What we aim for is not to bring one single area closer to an ideal. It is to bring the face as a whole closer to the impression the patient hopes for. This is the idea we call "whole-face design."

02 Verbalize the wish

Starting by putting your wish into words

What we do first in counseling is not talk about specific procedures. "I want to be cuter." "I want to change my stern impression." A vague wish is fine at first. Whole-face design begins by putting your feelings into words.

How Whole-Face Design Proceeds The Process of Total Face Design

  1. Step 1. Put the impression you want into words

We carefully unpack which features and balance of the face produce the impression behind each word you use: "cute," "mature," "soft." As we talk, you may discover an ideal you had not noticed yourself, or realize that "this part is beautiful just as it is."

  2. Step 2. See your current face from many angles

From the front, diagonal and profile, we check the skeleton, soft tissue, position of fat, and even the habits in how your expression moves. The relationship of nose, mouth and chin in profile and the contour line seen diagonally: the "three-dimensional balance" that cannot be seen from the front alone decides the beauty of the result.

  3. Step 3. Share the part we call "design sense"

Golden ratios and general standards such as "moving this by X millimeters makes it beautiful" are a matter of course for us doctors. But they alone do not decide everyone's beauty. How to finish what numbers and data cannot measure is where a cosmetic surgeon's true skill shows. We call this area "design sense," and we are more confident in it than anyone. While explaining our thinking carefully, we adjust even the finest details until the result matches the image you have in mind.

03 As a team

Seeing the whole face, as a team

What supports whole-face design at KAI CLINIC TOKYO is a team of strong doctors.

Every doctor on staff is a board-certified plastic surgeon with thorough knowledge of facial anatomy, so every one of them can handle the eyes, nose and contour. On top of that, each doctor has a distinctive specialty of their own.

A doctor who examines the foundation of the face centered on osteotomy, a doctor internationally recognized for rhinoplasty, a doctor with more than 10,000 eye procedures: top-class strengths of each kind are gathered in one clinic.

A consistent treatment system that looks at the whole face

In counseling, the doctor in charge at your first visit diagnoses the whole face and proposes the best treatment plan. Even when you wish to have surgery on multiple areas, as a rule one doctor operates from start to finish and balances the whole face beautifully. However, if another doctor in our clinic is better suited to your request, we may introduce that doctor to you.

What we aim for is aesthetic medicine that does not rest on individual skill alone.
Combining each doctor's strengths, and always pursuing and delivering the highest level of quality as a team.
That is the style of KAI CLINIC TOKYO, and our promise to our patients.

04 Design across time

Including "the time beyond" in the design

There is one more thing we include in whole-face design: the "time axis" of the result.
  • Completion Swelling subsides,
    tissue settles
    and the result is complete
  • 5–10 Years Still natural
    after 5 or 10 years
  • Beyond Aging beautifully
    into the future
    beyond that
The result of cosmetic surgery continues not only at the moment the operation is finished, but into the future, five and ten years later. A design that looks "the best it has ever been" once the tissue has settled and a design that still looks natural after more years have passed do not necessarily coincide.

That is why we make the treatment plan by calculating the movement of expression, changes with aging, and how each feature will age in harmony with its surroundings.

What we value is "not a momentary beauty, but a result that stays natural for a long time." This never means keeping the change small. Even for a procedure that greatly changes the impression of the face, it means a sustainable design that endures the time beyond.

Not addition

Whole-face design is not
merely about "adding" procedures

When you hear "whole-face design," you may worry that you will be urged to have many procedures.

In aiming for harmony of the whole face, proposing several procedures can sometimes be best. But what we value is not adding procedures indiscriminately.

Why is that procedure needed, and how far should we change things to reach the ideal? We make the priorities clear, and we frankly tell you that unnecessary or low-priority procedures are "not needed for now."

Not to increase the number of proposals, but to find together a "well-grounded combination" that brings you closer to what you hope for. That is whole-face design as KAI CLINIC TOKYO sees it.
